How do I make a claim?

Step 1: Contact us

Call us on 0800 108 108 (Mon to Fri, 9am to 8pm NZST) or email us at claims@momentumlife.co.nz to let us know you will be making a claim.

Your dedicated Claims Manager will talk you through the claims process and provide you with the necessary forms. You can also download a claim form from the links below:

Step 2: Send us all necessary details

Please complete the forms provided to you and return to us with the necessary documentation by mail.
